Why we teach what we teach

This link shows why we teach Tap, Rack and assess when you have a malfunction. Part of Tap, Rack and Assess is lifting the gun to visually inspect the chamber to assure the firearm is in battery. If the shooter had visually assessed the gun after racking he would have seen the problem and avoided a very expensive and painful accident.
